allows nurses to deliver crucial mental health support and interventions
directly within the community, positively influencing patients' lives. The

The ability to provide compassionate, patient-centered care on a flexible

accommodating. Main duties of the job Providing robust assessment of
patients (by telephone or face-to-face) who have been referred due to
presenting with mental health difficulties. Identifying appropriate care pathways
and ensuring the least restrictive option to meet the needs of the patient and
ensuring their wellbeing. Signposting to appropriate care
providers (including primary and tertiary care). Raising concerns to senior staff for
support and management. Completing documentation and
planning care for patients. Presenting cases to
Multi-Disciplinary Team for discussion around care pathway. Liaison with colleagues nursing,
