We are looking for a skilled, enthusiastic, and experienced Clinical Pharmacist to join the Lewisham Division of Modality Partnership on a full-time contract for 37.5 hours per week. In this role, you will be instrumental in delivering high-quality, patient-centred pharmaceutical care, optimising medicines safely and effectively, and driving innovation in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions. Key responsibilities include: Clinically assessing, diagnosing, and prescribing (where qualified) Optimising medicines and improving prescribing safety Collaborating with a multidisciplinary team to enhance patient outcomes Supporting medication reviews and structured medicines optimisation Contributing to service development and innovative working practices Working with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities This role requires innovation, flexibility, and commitment, offering the opportunity to work resourcefully within a team to meet organisational priorities while maintaining the highest standards of patient care.
