Main Responsibilities: As a valued member of Main duties of the job As a Pharmacist at Operose Health, you will work closely with clinicians to improve patient care and make cost-effective prescribing decisions. Your responsibilities include running daily telephone triage clinics, reviewing pathology results, leading repeat prescription services, and participating in home visits and immunization initiatives. You will also review and act on hospital correspondence, particularly regarding medicine reconciliation, and liaise with local pharmacies to enhance patient outcomes. Working within a dedicated team, you will contribute to transforming the patient experience in primary care. We seek a GPhC registered candidate with excellent communication skills, a can-do attitude, and a desire to improve patient care.
