Glebe Road Surgery Lead Practice Nurse The closing date is 01 November 2026 Apply for this job Job summary Glebe Road Surgery brings to you an exciting opportunity for a Senior Practice Nurse to join our very friendly nursing team to lead and support them in their roles. You will have experience as a Practice Nurse in primary care and will be expected to undertake all aspects of general practice nursing, including baby imms, chronic disease management and cervical screening. Main duties of the job To lead on Infection Prevention Control and work with the partners and practice manager to ensure CQC compliance at all times. Provide routine nursing care to patients in accordance with clinical based evidence, NICE and NSF Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing Understanding the importance of evidence-based practice Understand the requirement for PGDs and associated policy Ability to record accurate clinical notes Ability to work within own scope of practice and understanding when to refer to GPs Chaperone procedure Womens health (cervical cytology, contraception, etc.) Immunisations (routine, childhood and travel) Chronic disease management in Asthma , COPD, Diabetes. We are part of Sheen and Barnes PCN and work very closely with our neighbouring practices ensuring we provide the best care for our patients.
