We are looking for a compassionate, proactive and person-centred Social Prescribing Link Worker to join our team and support patients to access the services, resources and opportunities available within their local community. In this rewarding role, you will work closely with patients, their families and carers, healthcare professionals and local community organisations to provide personalised, holistic support. You will take the time to understand what matters most to each individual, helping them identify their goals and overcome social, practical and wellbeing challenges. You will support patients to develop personalised care and support plans and connect them with relevant local services, community groups and activities that can enhance their wellbeing, build confidence and improve their quality of life. Working as part of a supportive multidisciplinary team, you will play an important role in strengthening community connections, promoting social prescribing and helping to address health inequalities. You will have the opportunity to build meaningful relationships, empower individuals to take positive steps towards their wellbeing, and contribute to better outcomes for both patients and the wider community. Main duties of the job Provide personalised, person-centred support to patients, helping them identify their needs, goals and priorities. Connect patients with appropriate community services, voluntary organisations, local activities and other sources of support. Develop and review personalised care and support plans, encouraging patients to build confidence and independence. Work collaboratively with GPs, healthcare professionals, families, carers and community organisations. Support patients with social, emotional, practical and wellbeing-related challenges that may affect their health. Promote social prescribing and strengthen links between primary care and local community services. Maintain accurate, confidential records and monitor outcomes to demonstrate the impact of support provided. Help identify and address barriers to accessing services, contributing to reducing health inequalities. Participate in multidisciplinary team meetings, training and service development activities. About us We are a well-established training practice in a purpose built (newly refurbished) premises, located on the Haringey Green lanes. The practice's aim is to provide our patients with an excellent standard of patient-centered care, with clinicians and patients jointly involved in all aspects of planning and delivering care to meet patients' health and well-being needs. You will work with colleagues including a team of GPs, practice Nurse, Healthcare Assistant, Clinical Pharmacist, Physician Associate & MSK practitioner, administrative staff and a wide variety of other health professionals.
