Woodberry Practice Clinical Pharmacist The closing date is 23 October 2026 Apply for this job Job summary The Woodberry Practice is looking for an enthusiastic and experienced Clinical Pharmacist to join our friendly and supportive primary care team. This is a face-to-face, practice-based role at The Woodberry Practice, working directly with patients and alongside our GPs, pharmacy team and wider multidisciplinary team. The successful candidate will play an important role in medicines optimisation and patient care, helping to ensure patients receive safe, effective and appropriate treatment. Previous experience working within a GP surgery or primary care setting and experience using EMIS are essential for this role. We are looking for someone who is organised, proactive and confident working independently as well as collaboratively. You will have excellent communication skills, a patient-centred approach and the ability to effectively manage your own clinical workload. Main duties of the job The Clinical Pharmacist will provide face-to-face clinical pharmacy support to patients at The Woodberry Practice, working closely with GPs and the wider practice team. The role will include undertaking structured medication reviews, supporting patients with long-term conditions, medicines optimisation, medicines reconciliation following hospital discharge and responding to medication and prescribing queries. You will support safe and effective prescribing, identify and address medication-related risks, ensure appropriate medication monitoring is completed and provide advice to patients and healthcare professionals. The role will also involve reviewing clinical information, using EMIS and relevant clinical systems, and contributing to prescribing, patient safety and quality improvement work within the practice. You will be expected to manage your workload effectively while working collaboratively with the wider team to support high-quality patient care.
