Central London Community Health Trust Specialist Occupational Therapist The closing date is 15 October 2026 Apply for this job Job summary Join our innovative Community Ageing Well Service (CAWS) and make a meaningful difference to older people living with frailty, complex health needs and long-term conditions. As a Specialist Occupational Therapist, you will work within a supportive multidisciplinary team to help people remain independent, safe and confident in their own homes. You will use your specialist skills to assess needs, provide person-centred occupational therapy interventions and help people achieve what matters most to them. CAWS provides proactive, preventative and holistic care, working closely with GPs, community rehabilitation, social care, care homes, voluntary sector organisations and Integrated Neighbourhood Teams. Our work focuses on: Promoting independence, wellbeing and quality of life Reducing falls and functional decline Supporting admission avoidance Enabling people to remain safely at home This fixed-term maternity cover offers an opportunity to develop your specialist practice while contributing to a service that makes a positive difference to patients, carers and communities. Main duties of the job As a Specialist Occupational Therapist, you will: Complete comprehensive occupational therapy assessments for people with complex physical, cognitive and functional needs. Develop and deliver personalised treatment and rehabilitation plans based on individual goals and strengths. Assess and recommend appropriate equipment, adaptations and moving and handling solutions. Support people to maximise their independence, function, confidence and quality of life. Work collaboratively within a multidisciplinary team to develop coordinated, person-centred care plans. Provide specialist occupational therapy advice to patients, carers and other health and social care professionals. Contribute to falls prevention, frailty management and admission avoidance initiatives. Provide supervision, guidance and support to therapy assistants and junior colleagues, where appropriate. Contribute to service development, quality improvement and clinical governance. Build effective working relationships across health, social care and voluntary sector services to support joined-up care. You will have the opportunity to use and develop your clinical expertise while contributing to a service focused on prevention, independence and better outcomes for older people.
