Working within a multidisciplinary team, you will conduct structured medication reviews, optimise prescribing for long-term conditions, and support safe, evidence-based prescribing practices. Your role will involve working directly with patients to improve medication adherence, reduce poly pharmacy risks, and manage complex prescriptions. You will also assist in medicinalreconciliation, support chronic disease management, and contribute to clinicalaudits and quality improvement initiatives.
