to support people to live longer and healthier lives? Do you have an interest in frailty? Havering Liberty Primary Care Network is looking
to appoint a Social Prescriber who has an interest in working with older
individuals living with frailty or complex needs to live longer and healthier
lives. The post holder will be part of an already established Social
Prescribing team and will join our current Frailty Social Prescriber in
supporting our PCN-Aligned Community Team (PACT), which consists of an Enhanced Nurse and Nursing Associate. PACT
completes home visits to proactively undertake comprehensive geriatric assessments
and LTC reviews, delivering high-quality, personalised and anticipatory care to
promote rehabilitation and prevent deterioration. The Frailty Social
Prescriber will build a
strong
understanding of the services available to support these patients
and their carers holistically. They will work closely with the PCN multidisciplinary team, GP practices and voluntary, community and social enterprise organisations to meet the needs of patients, families and
carers. Main duties of the job The
post holder will support older people living with frailty to live longer,
healthier lives. They will work closely with Liberty PCNs five GP practices,
the wider PCN team, the local authority, including social care and housing, and
Voluntary Community and Social Enterprise
organisations to ensure individuals receive appropriate, coordinated
support. The
Frailty Social Prescriber will assist patients whose care is coordinated by the
PCN Aligned Community Team (PACT). PACT serves as a single point of contact for
individuals and carers, coordinating the provision of comprehensive, integrated
care in collaboration with other services and professionals, such as specialist
community teams, social prescribers, voluntary sector organisations, and
primary care. The responsibility of the Frailty Social Prescriber is to develop
an understanding of services that can support patients and carers from a
holistic perspective. The post holder will work with individuals from various cultural
and social backgrounds. The ability to perform effectively in a diverse and
occasionally demanding environment is required. About us We are a collaborative neighbourhood
healthcare network of five General Practices serving the population with an
extended multi-disciplinary team seeking to provide accessible,
patient-centred, and high-quality primary care services that promote the health
and well-being of our community and our workforce. The PCN supports numerous innovative
projects, looking to improve the health and well-being of our community, e.g. Social
Prescriber projects supporting High Intensity Users, bereaved family/friends of
recently deceased patients and identifying carers to complete welfare checks. The
Leadership Team of Havering Liberty PCN is headed by the Clinical Director. This
team includes the Strategy and Transformation Manager and the PCN Operations
Manager and Digital Facilitator working in collaboration with Leads for each
practice within the PCN. The PCN receives
