Main duties of the job Deliver High-Quality Care:Support patients with neuro conditions, including stroke, to enhance independence and quality of life. Collaborate Across Teams:Work closely with multidisciplinary colleagues to implement integrated care plans. Professional Development:Engage with CLCH Academy programs for CPD, skill enhancementand clinical academic opportunities. Mentorship & Team Support:Share knowledge, support colleagues and contribute to continuous learning within the team. Community Visits:Provide care in the community asrequired. Car access is essential unless exempt under the Equality Act 2010, with appropriate insuranceand documentation. Record Keeping:Maintainaccuratepatient records and reports in line with professional and organisational standards. Flexible Working:Participate in weekend work on a rota of one weekend every six weeks .
