We are seeking an enthusiastic and highly organised Clinical Flow Coordinator to join our Care Coordination Centre (CCC) based at St Margaret's Hospital, Epping. This is an exciting opportunity to play a pivotal role in supporting system-wide patient flow across West Essex, working closely with multidisciplinary teams, community services, acute providers, local authorities and wider health and care partners. The role focuses on coordinating safe transfers of care, supporting discharge processes, monitoring patient pathways and helping to manage operational pressures across the health and care system. This is a fixed-term contract until 31 March 2027 with the potential for extension, working 37.5 hours per week, Monday to Friday. There are 2 positions available, 1 full time and 1 part time (22.5 hours). We are looking for an individual who thrives in a fast-paced environment, has excellent communication and organisational skills, and can work collaboratively with a wide range of professionals. You'll be joining a supportive and innovative team that is committed to delivering outstanding care and improving outcomes for patients across the system. Main duties of the job As a Clinical Flow Coordinator, you will play a key role in supporting the effective operation of the Care Coordination Centre (CCC) and Transfer of Care Hub, helping to improve patient flow across the West Essex health and care system. Working closely with clinicians, operational managers and partner organisations, you will support admission avoidance, discharge planning and the safe transfer of care for patients. You will maintain patient flow trackers and operational systems, ensuring accurate and timely data to provide oversight of demand, capacity and patient outcomes. The role involves coordinating multidisciplinary activities, monitoring patient progression through care pathways, escalating delays or risks, and ensuring agreed actions are completed. You will also support referral screening, bed management processes, service improvement initiatives and the production of reports and information to support operational decision making. Working collaboratively with colleagues across community, acute, social care and voluntary sector services, you will help ensure patients receive the right care, in the right place, at the right time.
