Working autonomously, you will provide specialist diabetes care for patients with complex needs, including referral triage, clinical assessment, treatment optimisation, and home visits for housebound patients. You will work closely with primary care, acute services, and community teams to improve patient outcomes and promote evidence based practice. The role also involves providing education and support to patients, carers, and healthcare professionals, whilst contributing to service development and supporting colleagues within the team. This is an excellent opportunity for an experienced Diabetes Nurse looking to develop their specialist practice within a supportive and progressive service. Independent Nurse Prescribing is desirable but not essential. Main duties of the job Provide specialist diabetes care to patients with complex or poorly controlled diabetes across community and primary care settings. Assess, plan, deliver and evaluate individualised care plans to improve diabetes management and patient outcomes. Initiate, monitor and adjust diabetes treatments, including insulin and GLP-1 therapies, in line with national guidelines. Educate and empower patients, families and carers to manage diabetes effectively and prevent complications. Work collaboratively with GPs, practice nurses and multidisciplinary teams to deliver coordinated, high-quality care. Provide specialist clinical advice, training and mentorship to healthcare professionals across primary, community and secondary care. Support the delivery of structured diabetes education programmes, including DESMOND. Lead and contribute to clinical audits, service evaluations and quality improvement initiatives to enhance diabetes services. Develop and implement initiatives to improve diabetes management, promote health and reduce complications. Maintain accurate clinical records and ensure compliance with evidence-based guidelines and Trust policies. Contribute to health promotion, patient education and ongoing service development.
