We are seeking an enthusiastic Practice Nurse to join our friendly, supportive, and forward-thinking team. This is an exciting opportunity to play a key role in delivering high-quality patient care, promoting healthy lifestyles, and supporting individuals to achieve the best possible health outcomes while helping shape the future of our practice. Working as part of a dedicated multidisciplinary team, you will bring previous experience in practice nursing and a sound understanding of the general practice environment. You will be a skilled and compassionate clinician, able to demonstrate competence in long-term condition management, childhood and routine immunisations, travel vaccinations, wound care, and cervical screening. This is a 25-hour per week position, working Monday, Thursday, and Friday, 9:00am to 5:00pm, with participation in a monthly late-night clinic until 8:00pm. Main duties of the job As a valued member of our clinical team, you will provide high-quality nursing care to a diverse patient population, delivering services such as long-term condition management, childhood and adult immunisations, cervical screening, wound care, and health promotion. You will play an active role in chronic disease monitoring, QOF achievement, and clinical audit activities, working collaboratively with the wider multidisciplinary team to improve patient outcomes, support preventative healthcare initiatives, and deliver excellent primary care services to our local community. About us Glyndon Medical Centre is a well-established, three-partner PMS practice operating across two sites in Woolwich and Plumstead. Serving a diverse population of over 7,000 patients, we are committed to delivering high-quality, compassionate, patient-centred care and working collaboratively to meet the needs of our local community. Working closely with our PCN colleagues, the local ICB and neighbouring practices, our experienced multidisciplinary team consists of 3 GP partners, 3 salaried GPs, 2 practice nurses, 2 clinical pharmacists, a physiotherapist, a social prescriber and a mental health nurse. Our clinical team is supported by an established and highly committed management, reception and administrative team. We are a small, friendly, and close-knit practice that values teamwork, innovation, and continuous improvement.
