Main duties of the job Your responsibilities will include: Managing and supporting patients with long-term conditions, including diabetes, asthma, COPD and hypertension Delivering cervical screening (smear tests) Providing childhood and adult immunisations and vaccinations Undertaking NHS Health Checks Wound care, dressings and suture removal Delivering health promotion and lifestyle advice Supporting patients to manage their health and wellbeing Contributing to QOF, enhanced services and quality improvement initiatives Maintaining high standards of infection prevention and control Working collaboratively with the wider practice team to achieve the best outcomes for patients About us Located in the heart of Clapham Old Town, just moments from Clapham Common, Grafton Square Surgery is a well-established, patient-focused GP training practice serving a diverse and vibrant local community.
