Central London Community Health Trust Occupational Therapist The closing date is 01 November 2026 Apply for this job Job summary Join CLCH Hertfordshire Adult Community Services as an Occupational Therapist on the Inpatient Rehabilitation Wards at Potters Bar Community Hospital. As part of a multi-disciplinary team, you will support patients with a variety of conditions through comprehensive occupational therapy assessment, treatment and discharge planning. In this Band 5 role, you will: Work collaboratively with colleagues to deliver patient-centred rehabilitation. Assess individual needs and plan appropriate occupational therapy interventions. Support safe, effective discharge planning. This is an opportunity to develop your practice while contributing to rehabilitation that helps patients achieve the best possible outcomes. Main duties of the job As an Occupational Therapist within the bed-based unit, you will: Provide occupational therapy assessment and treatment for referred patients, planning, delivering and evaluating interventions for individuals and groups. Manage your own caseload autonomously in line with professional standards and clinical guidelines, maintaining accurate professional documentation. Contribute actively to the multi-disciplinary team, delivering a comprehensive Occupational Therapy service for residents of West Hertfordshire. Build and maintain effective relationships with referring CLCH acute and community services to support appropriate referrals and understanding of the Occupational Therapist and multi-professional team roles. Take part in Continuing Professional Development, in-service training, audit and research undertaken by the service.
